Quick answer: The most effective treatments for acne scars on Indian skin are fractional CO2 laser, microneedling (dermapen), and chemical peels, used alone or in combination depending on scar type. Most patients need three to six sessions. A dermatologist assessment is the first step to choosing the right plan for your skin.
Acne scars are not the same problem for every patient. Some people have shallow discolouration that fades with time and a good sunscreen. Others have deep pits or textural changes that have been there for years. The treatment that works for one does not work for the other.
This guide explains the different types of acne scarring, which treatments are most effective for Indian skin, and what you can realistically expect from a treatment course at DermaVue.

Acne Scars vs Acne Marks: An Important Distinction
Before any treatment discussion, this distinction matters.
Acne marks (post-inflammatory hyperpigmentation or PIH) are flat discolourations, red or brown, left behind after a pimple heals. They involve no change in skin texture. They are not true scars. Most PIH fades on its own over three to twelve months with consistent sunscreen use and, in some cases, a brightening agent like niacinamide or azelaic acid.
Acne scars involve a permanent change in skin texture or structure. They result from the skin’s repair process after deeper acne inflammation. Once formed, they do not resolve on their own.
If your concern is flat dark marks rather than textural changes, the treatment approach is different and generally simpler. If you are unsure which you have, a dermatologist can confirm at a quick assessment.
Types of Acne Scars
Understanding scar type is essential because different scars respond to different treatments.
Atrophic scars (depressed scars) are the most common. They form when the skin loses tissue during healing.
| Scar type | Appearance | Depth |
|---|---|---|
| Icepick scars | Narrow, deep pits, like a puncture | Deep |
| Boxcar scars | Wide, defined edges, flat base | Moderate to deep |
| Rolling scars | Broad, wave-like undulation, soft edges | Moderate |
Hypertrophic and keloid scars are raised, caused by excess collagen production. More common on the chest, back, and shoulders. Less common on the face.
Most patients have a mix of scar types, which is why a combination approach often gives the best results.
Treatment Options: What Works and What to Expect
Chemical Peels
Chemical peels use controlled concentrations of acid (glycolic, salicylic, TCA, or combinations) to remove the outer layers of skin and stimulate new collagen production.
What they treat: Superficial acne scars, post-inflammatory hyperpigmentation, uneven texture.
What to expect: Mild peeling for two to five days. No significant downtime for superficial peels. Medium-depth peels (TCA) have more downtime but produce deeper results.
Sessions needed: Three to six sessions, spaced three to four weeks apart.
Suitable for Indian skin: Yes, but the acid concentration must be calibrated carefully. Stronger peels on darker skin without proper preparation can worsen pigmentation. At DermaVue, all peels are selected based on your Fitzpatrick skin type.
Microneedling (Dermapen)
Microneedling uses a device with fine needles to create controlled micro-injuries in the skin. This triggers the skin’s natural healing response and stimulates new collagen and elastin production.
What it treats: Rolling scars, boxcar scars, general textural irregularity, skin laxity. Also useful for PIH.
What to expect: Mild redness for 24 to 48 hours. No significant downtime. Skin feels smoother and more even progressively over the treatment course.
Sessions needed: Four to six sessions, spaced four weeks apart.
Suitable for Indian skin: Microneedling has a low risk of post-inflammatory hyperpigmentation compared to laser, making it a preferred starting point for darker skin tones. It can also be safely combined with PRP (platelet-rich plasma) or GFC (growth factor concentrate) for enhanced collagen stimulation.
Fractional CO2 Laser
Fractional CO2 laser delivers controlled columns of laser energy into the skin, creating microscopic zones of thermal damage surrounded by untreated tissue. The treated zones resurface and the untreated zones support rapid healing. The result is significant skin remodelling with less downtime than fully ablative laser.
What it treats: Icepick scars, boxcar scars, deep rolling scars, severe textural irregularity. Most effective treatment available for moderate to severe atrophic acne scarring.
What to expect: Redness and swelling for three to five days. Skin feels rough and then peels over the following week. Full results take three to six months per session as collagen remodels. Strict sun avoidance is essential during recovery.
Sessions needed: Three to five sessions, spaced six to eight weeks apart.
Suitable for Indian skin: Yes, with appropriate settings. Indian skin needs lower fluence, higher density settings, and careful pre-treatment priming with a depigmentation protocol to reduce PIH risk. At DermaVue we use US-FDA approved fractional CO2 laser with settings calibrated for Fitzpatrick types IV to VI.
Subcision
Subcision is a minor in-clinic procedure where a fine needle is inserted under a depressed scar and moved side to side to break the fibrous bands that pull the scar downward. This releases the tethering and allows the depressed area to rise.
What it treats: Rolling scars and tethered boxcar scars specifically. Often combined with microneedling or filler for enhanced results.
What to expect: Minor bruising for three to five days. No significant downtime.
Sessions needed: One to three sessions.
TCA Cross
TCA CROSS (Chemical Reconstruction of Skin Scars) is a technique where high-concentration trichloroacetic acid is applied precisely to the base of individual icepick scars using a fine applicator. This causes focal collagen production at the base of the scar, gradually raising the floor.
What it treats: Icepick scars specifically. Standard resurfacing treatments have limited effect on true icepick scars because of their narrow depth.
What to expect: Treated spots form a crust that falls off over one to two weeks. Gradual improvement over three to four months per session.
Sessions needed: Three to six sessions, spaced six to eight weeks apart.
Dermal Fillers for Scars
For isolated deep boxcar or rolling scars, a small amount of hyaluronic acid filler placed under the scar can immediately raise the depressed area and improve appearance. This is not a permanent correction but can give immediate improvement while laser or microneedling treatment takes effect.
What it treats: Individual deep depressed scars.
What to expect: Immediate visual improvement. Filler lasts six to eighteen months depending on the product used.
Choosing the Right Treatment Plan
No single treatment works for all scar types. Most patients at DermaVue follow a staged plan:
| Stage | Typical approach | Purpose |
|---|---|---|
| 1 | Chemical peels or superficial microneedling | Address PIH, improve overall skin quality, prepare for deeper treatment |
| 2 | Microneedling with PRP or GFC | Stimulate collagen for rolling and boxcar scars |
| 3 | Fractional CO2 laser | Target deeper scars, drive significant remodelling |
| 4 | TCA CROSS or subcision (where needed) | Address residual icepick or tethered scars |
| 5 | Maintenance | Sunscreen, topicals, annual top-up sessions |
The exact plan depends on your scar types, skin tone, downtime tolerance, and budget. This is determined at the initial consultation.
A Note on Indian Skin
Indian skin (Fitzpatrick types IV to VI) responds differently to resurfacing treatments than lighter skin tones. The main risk is post-inflammatory hyperpigmentation: the skin responds to trauma with excess melanin production, which can temporarily worsen the appearance.
This does not mean aggressive treatments cannot be used. It means they must be used with:
- Proper skin preparation: a depigmentation protocol (often including azelaic acid, kojic acid, or a low-strength retinoid) started four to six weeks before treatment
- Conservative settings on the first session, adjusted based on response
- Strict sun avoidance after every session
- A dermatologist experienced in Indian skin
At DermaVue, all acne scar protocols are designed for Indian skin. We do not use Western default settings on Fitzpatrick type IV to VI skin.
What Does Not Work (and What to Be Cautious About)
Home devices: Derma rollers sold online use shorter needles than clinical microneedling devices and are not sterilised to clinical standards. They can cause infection and irregular results. They are not equivalent to clinical microneedling.
Fairness creams marketed for scars: Most over-the-counter brightening products do not penetrate deeply enough to affect true scars. They may help with superficial PIH over many months but will not change scar texture.
Aggressive peels at salons: High-concentration chemical peels performed without medical supervision carry a significant risk of chemical burns and permanent hyperpigmentation on Indian skin.
Single-session guarantees: Any clinic offering complete acne scar removal in one session is overpromising. Collagen remodelling is a biological process that takes months. No single session produces full correction.
