Fatty Liver Treatment in Kerala: Stage It, Reverse It
Close to half the adults screened in parts of Kerala carry fatty liver, most of them without a single symptom. This is a doctor-led guide to what the diagnosis means, how we stage it, and what the evidence actually supports, written for Indian patients by Dr. Rejeesh Menon, MD Internal Medicine, FACP.
Before you read on. If you have yellowing of the eyes or skin, a swollen abdomen, vomiting of blood, black tarry stools, or new drowsiness or confusion, do not wait for a consultation. These can be signs of advanced liver disease. Go to a hospital emergency department now or call 112.
Fatty liver, now called MASLD, is often manageable, and early-stage fat frequently improves with sustained weight loss of around 7 to 10 percent plus metabolic treatment. Advanced scarring (fibrosis) is managed to slow progression, not reversed. DermaVue physicians in Kerala stage your liver with FIB-4, ultrasound and FibroScan before any plan.

What fatty liver (MASLD) is, and what changed in the 2023 rename
Fatty liver means fat has built up inside the liver cells. When that happens alongside a metabolic problem, such as a large waist, high blood sugar, raised triglycerides or high blood pressure, doctors now call it MASLD, short for metabolic dysfunction-associated steatotic liver disease. In 2023 a global panel of liver societies retired the old name, non-alcoholic fatty liver disease (NAFLD), and replaced it with MASLD (Rinella 2023). The change was not cosmetic. The old name defined the disease by what it was not, that is, not caused by alcohol. The new name defines it by what it is: a metabolic condition.
The same consensus added a sensible middle category called MetALD, for people who have the metabolic pattern and also drink a moderate amount (above roughly 140 grams of alcohol a week for women and 210 for men). In Kerala, where toddy and social drinking are part of daily life for many, that category matters. It lets a doctor name both drivers honestly instead of blaming one.
Grade 1, grade 2, grade 3: what your ultrasound report actually says
Indian ultrasound reports usually grade fatty liver from 1 to 3 by how bright the liver looks on the scan. Patients read "grade 2 fatty liver" and assume it is a severity verdict. It is not the whole story. The grade measures fat, not scar. Scarring, which doctors call fibrosis, is the thing that actually predicts whether a liver will cause trouble decades later. A liver can be bright with fat (grade 3) and still have very little scarring, while a less fatty liver (grade 1) with high fibrosis markers is the one to worry about. The scan estimates these grades from the brightness of the liver, so the fat percentages below describe the amount of fat each grade corresponds to, not a figure the ultrasound measures directly.
Fatty liver symptoms: why it is usually silent
Most people with a fatty liver feel completely well. There are often no symptoms at all, which is why it is usually found by chance on a scan done for something else, where the report may call it "fatty infiltration of the liver." When symptoms do appear they are vague: tiredness, a dull ache or a heavy feeling in the upper right side of the abdomen, sometimes poor concentration. The clear warning signs, yellow eyes or skin, a swollen abdomen, or vomiting blood, belong to advanced disease and are a reason for urgent care, not a routine visit. Because the early disease is silent, the causes matter more than the symptoms: extra weight around the middle, high blood sugar or diabetes, high triglycerides, and a diet heavy in refined rice and sugar. If those apply to you, feeling fine is not reassurance; a FIB-4 score is.
The fatty liver spectrum, staged honestly: is fatty liver reversible?
Early fatty liver often improves a great deal. Simple fat and early inflammation respond well to losing around 7 to 10 percent of body weight and treating the metabolic drivers. In a 52-week study of 293 patients, losing 10 percent or more of body weight resolved steatohepatitis (the inflamed stage, called MASH) in about 90 percent of people and improved fibrosis in about 45 percent (Vilar-Gomez 2015). Advanced cirrhosis (stage F4) is a different matter: it is not considered reversible, and the aim there is to halt progression and protect what is left.
So the spectrum runs in stages. It starts as simple steatosis (fat only). In some people it becomes MASH, where the fat is joined by inflammation and cell injury. Over years, MASH can lay down fibrosis, scored from F1 to F4. The jump that changes a life is not how much fat there is, it is how much scar.
Why fibrosis stage, not fat, decides the outcome
This is the single most useful idea on this page, and it is missing from most fatty liver articles. The amount of fat on a scan correlates weakly with long-term liver outcomes. The fibrosis stage correlates strongly. That is why a good clinic does not stop at "you have a fatty liver." It asks the next question, which is: how much scarring, if any, has formed? Answering that needs a score and sometimes a scan, not a guess.
Fibrosis stage, not the amount of fat, is what predicts long-term liver outcomes.
Why Indians get fatty liver below a BMI of 25: lean MASLD and the thin-fat body
One of the most common things we hear is, "But I am not even overweight." You do not have to be. Indians develop fatty liver at a lower body weight than Western populations, a pattern called lean MASLD. The reason is the South Asian thin-fat body type: for the same BMI, Indians carry more fat around the internal organs (visceral fat) and have more insulin resistance, even when the arms and legs look slim. Gene variants common in South Asians, such as PNPLA3 and TM6SF2, add to the risk.
This is exactly why DermaVue uses Indian cut-offs, not the older international ones. For Indians, overweight begins at a BMI of 23 and obesity at 25 (Misra 2009), along with a waist above 90 cm in men and 80 cm in women. A person at a BMI of 24, considered a healthy weight by the old charts, can already be carrying a metabolically loaded, fatty liver. Reading that patient as low-risk is a mistake, and it is a mistake Western-calibrated tools make often.
How DermaVue stages your liver: FIB-4, ultrasound and FibroScan
You cannot stage your own liver from a report that says "grade 2 fatty liver," and neither can a weighing scale. Staging is a short, mostly non-invasive sequence that tells us the one thing that matters: how much scarring, if any, has formed.
FIB-4: the free first-line score from a blood panel you may already have
FIB-4 is the starting point because it is cheap, validated and often free. It is calculated from four values that sit on an ordinary blood report: your age, two liver enzymes (AST and ALT) and your platelet count. No new blood draw is usually needed. One honest caveat for Indian patients: the standard 1.3 cut-off was derived in Western cohorts, and Indian diabetes data suggest it can under-detect scarring here, so we read it alongside your waist, sugars and risk factors rather than as a lone verdict.
| FIB-4 score | What it suggests | What we do next |
|---|---|---|
| Below 1.3 | Low risk of advanced fibrosis | Advanced scarring is unlikely. Treat the metabolic drivers and recheck on a schedule. |
| 1.3 to 2.67 | Indeterminate | A FibroScan (transient elastography) sorts out who needs closer attention. |
| Above 2.67 | Higher risk of advanced fibrosis | Elastography and a specialist liver review are warranted. |
Ultrasound and FibroScan: seeing the fat, then measuring the stiffness
A standard ultrasound confirms and grades the fat (that grade 1 to 3 scale). When the FIB-4 lands in the grey zone, or when diabetes or a high waist raises the stakes, the next test is transient elastography, better known as a FibroScan. It is a quick, painless probe that measures liver stiffness; a stiffer liver means more scarring. A full stomach or a lot of body fat can push the reading up falsely, so it is done fasting and sometimes with a larger XL probe. The table below shows how the ultrasound grade maps to fat, and why it is not the same as your scar score.
| Ultrasound grade | How much fat | What it means (and does not mean) |
|---|---|---|
| Grade 1 (mild) | Fat in roughly 5 to 33 percent of liver cells | The echo pattern is slightly brighter than normal. This is a fat measurement, not a scar measurement. |
| Grade 2 (moderate) | Fat in roughly 34 to 66 percent of liver cells | Brighter liver, the vessel walls start to blur on the scan. Still describes fat, still says nothing about fibrosis. |
| Grade 3 (severe) | Fat in more than 66 percent of liver cells | Markedly bright liver, the deep part of the organ is hard to see. More fat, but a low FIB-4 can still mean low scarring risk. |
The grade describes fat, not scarring. To see where your fibrosis risk actually sits, the FIB-4 calculator gives you a one-minute answer. A normal weight does not rule it out either, which is why we wrote about lean fatty liver in slim and normal-weight people.
The metabolic drivers and the Kerala plate
The Trivandrum NAFLD cohort followed 2,158 adults from the community, not a hospital, and found fatty liver in 49.8 percent of them, roughly double the global pooled figure of about 25 percent (BMJ Open 2019). A larger national effort, the CSIR-IGIB Phenome India cohort of 7,764 adults, put age-adjusted MASLD at 38.9 percent, with significant scarring in 6.3 percent of those affected. These are not small numbers, and Kerala sits near the top of the range.
Rice, tapioca, coconut and toddy: glycemic load, not blame
The Kerala plate is not the villain, and no doctor worth seeing will tell you to give up rice. The issue is glycemic load and total energy. A large mound of refined white rice, eaten three times a day, drives a sharp blood sugar spike that pushes the liver to make fat. The fix is not deprivation, it is sequencing and swapping: a smaller portion of rice, some of it replaced by a lower-glycemic grain, protein and vegetables added to the same meal to blunt the spike. Coconut in normal amounts is not the problem; the daily load of refined carbohydrate is. Toddy and regular alcohol add a second hit, which is where the MetALD category earns its place. Our Kerala fatty liver diet turns all of this into a practical plate, meal by meal.
Fatty liver and type 2 diabetes in Kerala
Fatty liver and diabetes travel together because they share one root: insulin resistance. In a small Kerala study of 220 patients on FibroScan, those with diabetes had higher mean liver-stiffness scores than those without (14.39 versus 8.14), illustrating locally what the larger Phenome India cohort also found, that scarring clusters in people with diabetes (Kunnath 2025; Phenome India 2026). The practical rule is simple. If you have diabetes, your liver deserves staging, which we cover in fatty liver and type 2 diabetes. If you have a fibrotic liver, your blood sugar deserves a proper look. The two conditions also share the insulin-resistance pathway that drives PCOS, which is why we assess them together.
Evidence-based management by stage, including the GLP-1 evidence
There is no magic remedy for fatty liver, and anyone who promises to "clear" or "cure" your liver is not being straight with you. What there is, is a stack of treatments with real evidence, applied according to your stage and your metabolic picture by a doctor who monitors the result.
Sustained weight loss, and the 7 to 10 percent threshold
Weight loss is the foundation, and the numbers are specific. Below about 5 percent of body weight, the liver changes little. The meaningful improvement in steatohepatitis turns on around 7 to 10 percent, and clear fibrosis regression needs closer to 10 percent. At 10 percent or more, the Vilar-Gomez study saw inflammation resolve in roughly 9 of 10 people and fibrosis improve in nearly half (Vilar-Gomez 2015). This is why a structured plan that reaches and holds a 7 to 10 percent loss does more for most livers than any supplement on the shelf. The calorie-deficit calculator can show you what that target looks like day to day.
Does Ozempic help fatty liver? What ESSENCE showed
For patients who also need medical weight management, the GLP-1 evidence is now hard to ignore. In the ESSENCE phase 3 trial of 800 biopsy-confirmed patients, semaglutide 2.4 mg resolved MASH without worsening fibrosis in 62.9 percent of people, against 34.3 percent on placebo, at 72 weeks; fibrosis improved in 36.8 percent versus 22.4 percent (Sanyal 2025, NEJM). That is a strong signal, and the Indian rules have now caught up with it. In July 2026 the drug regulator (CDSCO) approved Wegovy, the semaglutide 2.4 mg brand, for non-cirrhotic MASH with moderate-to-advanced fibrosis, the first GLP-1 cleared for a liver indication in India (Novo Nordisk announcement, July 2026). That approval is for a specific group, people with F2 to F3 fibrosis, taken alongside diet and activity, not a blanket option for everyone with a fatty liver. Whether it is right for you is a decision we make together, based on your stage and your whole metabolic picture. You can read the full picture on the medicines in our guide to medicine for fatty liver and the Ozempic and Wegovy guide.
Tirzepatide, SYNERGY-NASH, and where resmetirom stands in India
Tirzepatide, the dual-agonist behind Mounjaro, was tested in the phase 2 SYNERGY-NASH trial of 190 patients. MASH resolution rose with the dose, reaching 51.8, 62.8 and 73.3 percent across the three doses against 13.2 percent on placebo at 52 weeks; the fibrosis benefit was a secondary, nominal finding, so this is promising phase 2 evidence, not a settled answer (Loomba 2024). Our Mounjaro guide covers it in detail. Resmetirom (Rezdiffra) became the first drug approved anywhere specifically for MASH when the US FDA cleared it in March 2024, but it is not marketable in India yet; the regulator has asked for bioequivalence and trial data first, so we mention it for completeness, not as something we can prescribe here.
| Option | Evidence | What it showed | India status |
|---|---|---|---|
| Sustained weight loss (lifestyle) | Vilar-Gomez 2015, Gastroenterology (n=293) | Losing 10 percent or more of body weight resolved steatohepatitis in about 90 percent of patients and improved fibrosis in about 45 percent. | The foundation of every plan. Available to everyone, no prescription needed. |
| Semaglutide 2.4 mg | ESSENCE phase 3, NEJM 2025 (n=800) | MASH resolved without worsening fibrosis in 62.9 percent on semaglutide versus 34.3 percent on placebo at 72 weeks. Fibrosis improved in 36.8 percent versus 22.4 percent. | As of July 2026, Wegovy (the semaglutide 2.4 mg brand) is CDSCO-approved in India for non-cirrhotic MASH with moderate-to-advanced fibrosis, taken with diet and activity. It is the first GLP-1 cleared here for a liver indication. It is for a specific group (F2 to F3 fibrosis), not everyone with a fatty liver, and eligibility is a physician decision. |
| Tirzepatide | SYNERGY-NASH phase 2, NEJM 2024 (n=190) | MASH resolution rose with dose, reaching 51.8, 62.8 and 73.3 percent across the three doses versus 13.2 percent on placebo at 52 weeks. The fibrosis signal was a secondary, nominal finding. | Phase 2 MASH evidence, available in India for weight and diabetes. Its metabolic and weight benefits still help the liver, used under physician guidance. |
| Resmetirom (Rezdiffra) | US FDA accelerated approval, March 2024 | The first drug approved anywhere specifically for MASH with moderate to advanced fibrosis. | Not marketable in India as of October 2026. The regulator has asked for bioequivalence and trial data before any Indian approval. We mention it for completeness, not as an option we can prescribe here. |

Exercise: the half of lifestyle that is not the plate
Diet gets most of the attention, but movement pulls its own weight. Both aerobic activity and resistance work lower liver fat even before the scale moves, and they add to what the diet changes achieve rather than competing with them. The target is regular activity on most days of the week, built into a routine you will actually keep, not a punishing programme you drop after a month. For a working adult in Kochi that often means a brisk 30 to 40 minute walk most evenings plus two short sessions of resistance work, which is enough to start shifting the visceral fat that drives the Indian pattern.
Home remedies, Ayurveda and "liver detox": what the evidence says
No tea, tablet or tonic has been shown to reverse a fatty liver. Milk thistle (silymarin) has not changed liver outcomes in controlled trials, "liver detox" products do nothing for fat or scarring, and papaya-leaf extract is used to raise platelet counts in dengue, not to treat the liver. There is a more serious point here. Several Ayurvedic and herbal "liver tonics" sold in India have themselves caused drug-induced liver injury, some of it severe enough to need a transplant, so an unproven supplement is not a harmless choice (Philips 2019). The treatment that actually works is unglamorous and well-proven: reach the 7 to 10 percent weight loss above, treat the metabolic drivers, and move more. If you are taking anything for your liver, tell your doctor exactly what it is and bring the box.
What a DermaVue fatty liver consultation looks like
A fatty liver consultation at DermaVue is a clinical assessment, not a sales pitch. Because you cannot self-stage, the whole point of the visit is to find out where your liver actually sits, to build a plan that matches that stage, and to tell you honestly whether you need a liver specialist or whether we manage it from here.
- Stage the liver. We review or arrange a FIB-4 score, a liver ultrasound, and a FibroScan where the score or your risk factors call for it. We read them together, not in isolation.
- Find the drivers. Blood sugar, insulin resistance, lipids, blood pressure, waist and diet all get examined, using Indian cut-offs. We look for the diabetes and PCOS links too.
- Build the plan. A realistic target for sustained weight loss, a Kerala-adapted eating plan, and, where it is clinically appropriate, a discussion of metabolic medication. Medication and any program fee are discussed in the consultation, not on this page.
- Follow up and re-measure. The only honest way to know a plan is working is to re-measure. We set a follow-up cadence, including repeat elastography where it is warranted.
Telehealth across South India: consult, test and treat from home
You do not have to live near a clinic to be assessed and treated properly. DermaVue runs the whole fatty liver pathway as a telehealth service across Kerala and Tamil Nadu, with clinics in Kochi, Thiruvananthapuram, Thrissur, Kollam, Kottayam, Thiruvalla and Coimbatore, and by secure video consultation anywhere else in South India.
A FibroScan, when it is needed, is arranged through a partner centre near you. Kerala also runs free government fatty liver clinics with FibroScan at Tirur District Hospital and the Thiruvananthapuram and Ernakulam General Hospitals, and we will tell you plainly when the free clinic is the right option for you.
