Physician Comparison Verified October 2026

Mounjaro vs Ozempic vs Wegovy: A Physician's Comparison for Indian Patients

Three names, two molecules, one decision. Here is how tirzepatide and semaglutide actually differ on trial weight loss, dosing, side effects and price in India, written to answer the question a patient is really asking: which one is right for me.

Bottom Line

All three are the same two molecules. Ozempic and Wegovy are semaglutide (Novo Nordisk); Mounjaro is tirzepatide (Eli Lilly). In the only head-to-head trial, SURMOUNT-5 (NEJM 2025), tirzepatide produced 20.2 percent mean weight loss versus 13.7 percent for semaglutide at 72 weeks. Wegovy is the obesity-approved option in India and the only one approved for fatty liver (MASH). On price, generic semaglutide starts at ₹325/week, Ozempic (Novo Nordisk) at ₹1,415/week, and Mounjaro (Eli Lilly) at ₹3,281/week.

The one trial that actually compared them

SURMOUNT-5: tirzepatide 20.2% vs semaglutide 13.7%

Most online comparisons quote weight-loss percentages from separate trials run in different patients. That is not a fair comparison. Only one published trial randomised these two molecules against each other in the same study: SURMOUNT-5 (New England Journal of Medicine, 2025), 751 adults with obesity and without diabetes, over 72 weeks.

Tirzepatide (Mounjaro) −20.2% mean body weight at 72 weeks
Semaglutide (Ozempic/Wegovy) −13.7% mean body weight at 72 weeks

In the same trial, 48.4 percent on tirzepatide lost at least 20 percent of body weight versus 27.3 percent on semaglutide. The honest caveats: the trial was open-label and sponsored by the maker of tirzepatide, it was not powered to compare safety, and individual response varies. Every other percentage on this page that compares drugs from different trials is labelled as not head-to-head, because it is not a fair comparison.

Weight-loss figures are the primary endpoints of each drug's pivotal trial in the population studied, not a promise of individual result, and only SURMOUNT-5 compared tirzepatide and semaglutide directly. Prices are the starting India MRP from the cost and pricing page; maintenance doses cost more.

Why tirzepatide and semaglutide differ

One receptor, two receptors, three receptors

The weight-loss difference is not marketing. It traces to how many gut-hormone receptors each molecule activates.

GLP-1 only

Semaglutide and liraglutide

Semaglutide (Ozempic, Wegovy, Rybelsus) and liraglutide (Saxenda) activate the GLP-1 receptor: glucose-dependent insulin release, slower gastric emptying, central appetite suppression and glucagon suppression. This is the established, most-studied mechanism.

GIP + GLP-1

Tirzepatide (Mounjaro)

Tirzepatide adds GIP-receptor agonism on top of GLP-1. GIP contributes to insulin sensitivity and lipid handling and appears to add to the appetite and energy effects, which is the mechanistic explanation usually given for the larger weight loss in SURMOUNT-5.

Glucagon + GIP + GLP-1

Retatrutide (investigational)

Retatrutide is a triple agonist that adds glucagon-receptor activity, which raises energy expenditure and drives liver-fat loss. Its phase-2 trial (NEJM 2023) reported 24.2 percent weight loss at 48 weeks. It is not approved anywhere and is not something DermaVue offers; it is included here only because patients ask what is coming next.

The evidence, by trial

What each drug has actually shown

Weight loss

Head-to-head, SURMOUNT-5 (NEJM 2025): tirzepatide −20.2% vs semaglutide −13.7% at 72 weeks. Not head-to-head: SURMOUNT-1 reported tirzepatide −20.9% at 72 weeks (15 mg, NEJM 2022); STEP 1 reported semaglutide 2.4 mg −14.9% at 68 weeks (NEJM 2021); SCALE reported liraglutide 3.0 mg −8.0% at 56 weeks (NEJM 2015). Those three are different trials in different patients and cannot be subtracted from each other.

Heart and organ outcomes (semaglutide's moat)

SELECT (NEJM 2023), 17,604 adults with established cardiovascular disease and without diabetes: semaglutide 2.4 mg cut major cardiovascular events by 20 percent (6.5% vs 8.0%, HR 0.80). FLOW (NEJM 2024) cut kidney and cardiovascular events by 24 percent in diabetic kidney disease. These are outcomes tirzepatide has not yet matched in published trials, which is why semaglutide is often the choice when the heart or kidneys are the priority.

Fatty liver, MASH (Wegovy's new indication)

ESSENCE (NEJM 2025): in biopsy-confirmed MASH with F2 to F3 fibrosis, semaglutide 2.4 mg resolved steatohepatitis in 62.9 percent versus 34.3 percent on placebo. This is the evidence base behind the July 2026 CDSCO approval of Wegovy for MASH in India, the first GLP-1 approved for a liver indication. It applies to F2 to F3 fibrosis on a physician decision, not as a self-serve use.

Sleep apnoea

SURMOUNT-OSA (NEJM 2024): tirzepatide reduced the apnoea-hypopnoea index by 55 to 63 percent in adults with obesity and obstructive sleep apnoea. A reason tirzepatide may be preferred when OSA is a major comorbidity.

What this means for Indian patients

The trial numbers were not measured in Indian bodies

India already carries a diabetes prevalence of 11.4 percent (about 101 million adults) and generalised obesity of 28.6 percent, per the ICMR-INDIAB study (Lancet Diabetes and Endocrinology, 2023). South Asians store more visceral and liver fat at any given BMI, with higher insulin resistance than matched Europeans. That is the thin-fat phenotype, and it is why India uses lower thresholds: overweight from a BMI of 23, obese from 25, not the WHO 25 and 30.

It also tempers the trial numbers. The pivotal trials reported 15 to 21 percent weight loss in largely non-Indian cohorts. India's first real-world data, a Max Hospital study of 150 patients in the Indian Journal of Endocrinology and Metabolism, showed a median of 8.2 percent at about six months, with 73 percent losing at least 5 percent and tirzepatide users reaching 10 percent loss sooner. The gap reflects shorter real-world follow-up, titration persistence and that higher baseline insulin resistance, not a failure of the drug.

One Indian-specific caution shapes how we prescribe either molecule: an Indian endocrinology consensus warns that rapid GLP-1 weight loss on a rice-dominant, often protein-light diet raises the risk of muscle and bone loss. That is why the program pairs the injection with a protein target and resistance training, rather than shipping a pen and leaving the rest to the patient.

Side effects and safety

Broadly similar, with a few real differences

Both molecules share the same side-effect family. The differences are in degree and in a handful of hard rules.

  • Stomach side effects are the common ones for all of them: nausea, early fullness, occasional vomiting, constipation or loose motions, mostly mild to moderate and worst in the first weeks of each dose step. Slow titration and diet changes reduce them.
  • Tolerance in the head-to-head trial: in SURMOUNT-5, stomach side effects led 2.7 percent of tirzepatide users and 5.6 percent of semaglutide users to stop. Overall discontinuation was close (6.1 vs 8.0 percent).
  • Muscle loss is part of total weight loss on any of these drugs, which matters more in the South Asian phenotype. Protein and resistance training are not optional extras.
  • Hard stops for both: a personal or family history of medullary thyroid cancer or MEN-2, a confirmed episode of pancreatitis, pregnancy or planning pregnancy within about two months, and breastfeeding. Tirzepatide can reduce the reliability of oral contraception, so a barrier method is advised for four weeks after starting and after each dose increase.
  • If you take insulin or a sulfonylurea for diabetes, those doses usually need reducing to avoid low blood sugar. That is a physician adjustment, not a self-managed one.

This is why these are physician-supervised treatments. The eligibility check below screens for the hard stops before anything else.

Which one, for whom

There is no single best drug, only the right fit

Cost is the main constraint

Generic semaglutide from ₹325 per week is the identical molecule to Ozempic and Wegovy. A vial needs a syringe and a little more care; many patients start on a pen for convenience and move to a vial later.

Obesity diagnosis or fatty liver

Wegovy is the semaglutide product actually approved for obesity in India, and the only GLP-1 approved for MASH (F2 to F3 fibrosis). If a diagnosis and indication matter, this is the on-label route.

Heart or kidney disease

Semaglutide has the outcome trials here: SELECT for cardiovascular events, FLOW for diabetic kidney disease. When the priority is protecting the heart or kidneys, the evidence points to semaglutide.

Largest trial weight loss, budget allowing

Tirzepatide (Mounjaro) led SURMOUNT-5 and helps obstructive sleep apnoea (SURMOUNT-OSA). It is the most expensive option in India and has no price cut, because it is the top-selling drug in the market.

Frequently Asked Questions

What patients ask when choosing between them

Which is better, Ozempic or Mounjaro, for weight loss?

In SURMOUNT-5, the only head-to-head trial of the two molecules (New England Journal of Medicine, 2025), tirzepatide (the molecule in Mounjaro) produced a mean weight loss of 20.2 percent at 72 weeks versus 13.7 percent for semaglutide (the molecule in Ozempic and Wegovy). On that trial, tirzepatide achieved greater average weight loss. It is also the more expensive option in India, and the right choice depends on comorbidities, tolerance and budget, which is a physician decision rather than a fixed ranking.

Is Mounjaro stronger than Ozempic?

On the available head-to-head evidence, yes for average weight loss. SURMOUNT-5 (NEJM 2025) randomised the two drugs directly and found tirzepatide 20.2 percent versus semaglutide 13.7 percent at 72 weeks. Tirzepatide acts on two receptors (GIP and GLP-1) while semaglutide acts on one (GLP-1), which is the mechanistic reason usually given. Note the trial was open-label and industry-sponsored, and individual response varies.

What is the difference between Ozempic and Wegovy?

Both are the same molecule, semaglutide, from Novo Nordisk. Ozempic is approved in India for type 2 diabetes and tops out at a 2.0 mg weekly dose. Wegovy is approved for obesity and titrates to 2.4 mg weekly, and since July 2026 it also carries a CDSCO approval for MASH (fatty liver with F2 to F3 fibrosis). When Ozempic is used for weight loss in India it is off-label; Wegovy is the obesity-indicated version of the same drug.

Is tirzepatide better than semaglutide?

For average weight loss in a head-to-head setting, SURMOUNT-5 showed tirzepatide ahead (20.2 vs 13.7 percent at 72 weeks). Semaglutide has the larger cardiovascular and organ-outcome evidence base in people without diabetes: SELECT (NEJM 2023) showed a 20 percent reduction in major cardiovascular events, and ESSENCE (NEJM 2025) showed MASH resolution. So the honest answer is that tirzepatide leads on weight and semaglutide leads on proven organ outcomes. The choice depends on what a given patient needs.

Which weight loss injection is cheapest in India?

Generic semaglutide is the lowest-cost option, from ₹325 per weekly vial dose (Natco Semanat, Glenmark GLIPIQ) after the March 2026 patent expiry, roughly ₹1,290 per month. Ozempic (Novo Nordisk) starts at ₹1,415 per week and Mounjaro (Eli Lilly) at ₹3,281 per week. Generic semaglutide is the identical molecule to Ozempic and Wegovy; the price gap reflects brand and pen-versus-vial format.

Which one is approved for obesity in India?

Wegovy (Novo Nordisk) is the semaglutide product approved by CDSCO for the obesity indication in India, and since July 2026 also for MASH with F2 to F3 fibrosis. Ozempic and Mounjaro carry type 2 diabetes approvals in India, so their use for weight management is off-label and physician-directed. Generic semaglutide vials are marketed for weight management by several manufacturers.

Can I switch from Ozempic to Mounjaro?

Switching between GLP-1 agents is done under physician supervision, not self-managed. A switch usually restarts at the new drug's lowest dose and titrates up on tolerance, because the drugs are not milligram-for-milligram equivalent. Reasons to switch include a weight plateau, side effects, or cost. The transition dose, timing and monitoring are set by the prescribing physician.

Do these injections work the same for Indian patients?

The pivotal trials (STEP, SURMOUNT) ran in largely non-Indian cohorts and reported 15 to 21 percent weight loss. India's first real-world data (a Max Hospital study of 150 patients in the Indian Journal of Endocrinology and Metabolism) showed a median of 8.2 percent at about six months, with 73 percent losing at least 5 percent. The gap reflects shorter real-world follow-up, titration persistence and the higher baseline insulin resistance of the South Asian thin-fat phenotype, which is also why India uses lower BMI thresholds (overweight from 23, obese from 25).
