What Cheiloplasty Is

A Hidden-Incision Reduction of the Lip's Inner Surface

The lip is built in layers: outer skin, orbicularis oris muscle, submucosa, and inner wet mucosa. The vermillion border is the sharp ridge where lip-red meets facial skin; the wet-dry junction lies inside the mouth where the moist mucosa begins. Lip volume in macrocheilia (constitutionally thick lips) comes predominantly from redundant wet mucosa and submucosal fibrofatty tissue, not from the muscle itself.

Cheiloplasty removes this redundant inner tissue, preserving the dry vermillion, the white roll, and the orbicularis bulk that gives the lip its natural pout. The incision sits entirely inside the oral cavity, posterior to the wet-dry junction. No incision crosses the vermillion border externally. Once healed, the scar is invisible to anyone looking at you and palpable only as a fine internal ridge.

Permanence is the trade-off. The excised tissue does not grow back. This is why Dr. Madhu KS marks each case conservatively (typically 1 to 2 mm under the patient's stated goal) and offers a secondary refinement at 6 months if more reduction is wanted. Going the other direction (adding volume after over-reduction) requires fillers and is harder to control.

You sit upright with lips at rest, then in animation. Dr. Madhu KS marks a transverse horizontal ellipse along the wet mucosa just inside the dry vermillion. Width of the ellipse is roughly 2 times the desired millimetric reduction (a 6 mm excision yields ~3 mm height reduction after closure). Symmetry is verified before any incision.

Regional nerve block, no general anaesthesia

Bilateral infraorbital block for the upper lip; bilateral mental block for the lower lip, using 2% lignocaine. Local infiltration with 1:200,000 adrenaline adds haemostasis. Onset 3 to 5 minutes. You stay awake and can talk between steps.

Symmetric ellipse excision

Full-thickness elliptical excision of the marked wet mucosa with a measured cuff of submucosal fibrofatty tissue. Symmetry is checked intra-operatively against the pre-op marks. For asymmetric or post-traumatic cases, a W-plasty pattern is substituted for the straight ellipse to distribute scar tension and avoid linear contracture.

Mucosa-to-mucosa closure

Interrupted 4-0 or 5-0 absorbable Vicryl, mucosa-to-mucosa. Deep submucosal bites first to obliterate dead space, then mucosal apposition. No skin closure needed because no skin is opened. Sutures absorb on their own in 10 to 14 days.

Discharge in 30 minutes

You walk out 30 minutes after the surgery finishes with a written aftercare plan, a chlorhexidine 0.2% mouthwash prescription, paracetamol for first-day soreness, and a direct WhatsApp line to the surgical team for the first 14 days.

Day 0

Can I get only the lower lip done, or both together?
Both are routine. Many Kerala patients present with a disproportionately thick lower lip and choose lower-only reduction. Combined upper and lower reduction in a single sitting is also common and avoids two recoveries. Dr. Madhu KS will recommend based on your photographs and on-table analysis.
Is lip reduction reversible if I do not like it?
The tissue removed cannot be put back surgically, so cheiloplasty is best understood as permanent. This is why conservative excision is the safer philosophy. Dr. Madhu KS typically excises 1 to 2 mm less than the patient's stated goal, with the option of minor secondary refinement after 6 months if needed.
Is there an age limit for lip reduction surgery?
Minimum 18 years to ensure facial growth is complete. There is no upper age limit provided you are medically fit. We routinely operate on patients in their 40s and 50s. A pre-operative medical review is done for anyone above 50 or with comorbidities.
Who is the right candidate for lip reduction?
Adults with constitutionally thick lips (macrocheilia), overfilled lips wanting to reverse hyaluronic filler effect, post-traumatic lip enlargement, or asymmetric lips after injury or infection.
