Insulin resistance cannot be seen on a standard fasting-sugar test, because sugar stays normal while insulin quietly climbs. The HOMA-IR test measures that hidden insulin directly.

What HOMA-IR is Two fasting values, one number

HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) turns your fasting glucose and fasting insulin into a single number that estimates how hard your body is working to keep sugar normal.

The formula is fasting insulin (micro-units per mL) multiplied by fasting glucose (mg/dL), divided by 405.

What the test involves One morning blood draw

One blood draw after an eight-to-twelve-hour overnight fast (உண்ணாநிலை). No glucose drink, no all-day testing.

A raised fasting insulin on its own is an early warning, even when the HOMA-IR looks borderline. Because it uses two routine assays, the test is inexpensive and widely available.

Lifestyle is the foundation for everyone. Medication is added only when the metabolic picture justifies it, and always under a physician after screening.

Metformin The long-established first medication

Metformin works mainly on the liver, quieting its excess glucose output, with a modest improvement in how muscle takes up glucose. It is first choice for insulin resistance and prediabetes when lifestyle alone is not enough, and it tends to trim visceral fat rather than add weight.

Common early effects are gastrointestinal, and long-term use can lower vitamin B12, which we monitor.

GLP-1 receptor agonists Physician-prescribed, screened, supervised

GLP-1 medicines such as semaglutide or tirzepatide amplify satiety, slow gastric emptying, and in studies reduce HOMA-IR, visceral fat, fasting sugar and triglycerides. They work alongside diet and exercise, not instead of them.
