Dandruff Treatment in Thrissur That Treats the Yeast, Not Just the Flakes
Dandruff is rarely about a dry scalp. On most Thrissur scalps it is Malassezia, a resident yeast that overgrows when the scalp's acidic protective film is disturbed by humidity, sweat and the soft, mildly acidic water most of central Thrissur washes with. Off-the-shelf shampoos quiet the flakes for a week. Dr. Shilpa and the DermaVue team in Punkunnam treat the yeast and the inflammation underneath, then hand you a maintenance plan so it stays controlled.

Dandruff treatment in Thrissur at DermaVue starts with scalp dermoscopy to separate simple dandruff, seborrheic dermatitis and scalp psoriasis, which look alike but need different drugs. Treatment is prescription antifungal (ketoconazole, ciclopirox or zinc pyrithione) plus an anti-inflammatory, then maintenance. Thrissur's 70 to 87% humidity and soft, weakly buffered local water keep Malassezia active, so recurrence is expected without a maintenance plan.
Why Dandruff Is Worse in Kerala's Monsoon Season
Dandruff and seborrheic dermatitis are driven by Malassezia, mainly M. globosa and M. restricta, two yeasts that live on almost every adult scalp without causing trouble. They feed on sebum: their lipase enzymes split scalp oils and release free fatty acids, chiefly oleic acid, that irritate a susceptible scalp and trigger the flaking, redness and itch. The scalp's defence against this is its acid mantle, a thin film that normally sits at pH 4.5 to 5.5. When that acidic set point is disturbed, Malassezia shifts from quiet resident to driver of inflammation. Measured scalp surface pH is consistently higher in people with seborrheic dermatitis, which is one reason the yeast gains the upper hand.
Two things in Thrissur keep the mantle unsettled. Warmth and humidity in the 70 to 87% range, with sweat sitting under the hair, give the yeast a warm lipid-rich surface to work on. Less obviously, the groundwater across much of central Thrissur, including the Punkunnam belt, is soft and only mildly acidic, around pH 5.5 to 6.5. Soft water carries little mineral buffering and rinses surfactant poorly, so every wash nudges scalp pH off its protective value and leaves shampoo residue behind. Repeated washing with weakly buffered water destabilises the mantle faster than it recovers. None of this causes dandruff on its own, but it loads the dice for anyone already prone to it.
This is why coconut oil, a Kerala scalp staple, can backfire on an inflamed scalp: it adds the exact fatty substrate Malassezia metabolises. It is also why so many Thrissur patients self-treat for months without progress. Simple dandruff, seborrheic dermatitis, scalp psoriasis and tinea (ringworm) of the scalp all flake, but each needs a different prescription. Scalp dermoscopy, with KOH testing where ringworm is possible, gives Dr. Shilpa a clear diagnosis before a single product is prescribed.
A 575-patient placebo-controlled trial published in the British Journal of Dermatology found 2% ketoconazole shampoo gave an excellent response in 88% of people with scalp seborrheic dermatitis and dandruff, with lower recurrence than zinc pyrithione 1%. Mechanism: Malassezia lipases and free fatty acids (NCBI) · The acid mantle (IJDVL).
Dandruff Treatment Options at DermaVue Thrissur
The aim is not a flake-free week. It is to lower the yeast load, calm the inflammation, and restore the scalp's acidic barrier so Malassezia stays in check long term.
Prescription Antifungal Shampoo
Ketoconazole 2% is first-line and well evidenced for seborrheic dermatitis. In a 575-patient placebo-controlled trial, 2% ketoconazole shampoo used twice weekly produced an excellent response in 88% of patients. Ciclopirox and zinc pyrithione are alternatives and useful for maintenance. Used 2 to 3 times weekly in the active phase.
Medicated Scalp Serums
Salicylic acid and antifungal leave-on serums lift adherent scale and lower yeast between washes, where a rinse-off shampoo gets little contact time.
In-Clinic Scalp Peels
Salicylic acid scalp peels clear thick, stuck-down scale so medication can actually reach the skin. Useful for severe or long-neglected seborrheic dermatitis.
Facial Seborrhoea Treatment
Eyebrows, sides of the nose, nasolabial folds and the beard area need creams and calcineurin inhibitors (tacrolimus, pimecrolimus), not shampoo.
Seborrheic dermatitis is chronic. Most patients reach good control in 4 to 6 weeks, then move to once or twice weekly maintenance. There is no permanent cure, but there is reliable long-term control. Topical antifungals can cause mild, temporary scalp dryness or irritation, and we adjust the regimen if that happens. If shedding has thinned your hair, we can add regrowth support once the scalp settles (PRP/GFC).
